Application Instructions

Complete the applicant demographic and optometric educational data.
Choose up to ten sites (there is a $10/site surcharge for each site over five) where you wish your application to be sent. This list is NOT for matching purposes. ORMS will forward your demographic and educational data to the programs listed in the second portion of the form.
Use the following link to obtain your OE Tracker number from NBEO: OE Tracker number.
Submit your application online by clicking on the submit button at the bottom of this page. Please submit only one application. If something changes or you wish to add another program this must be done via e-mail (orms@orms.org).
Should you wish to submit this application by fax or mail, then copy this page to your computer. Complete the application on your computer. Print the application and then submit as you desire.
Don't forget, to make your application official you must pay online or mail your application fee ($30 + $10 for every program over five for applications received by February 1; for applications after that date there is a $15 late fee assessed) to ORMS at 151 Principia Court, Claremont, CA 91711-4657.
Use the "Tab" key and "Shift-Tab" key combination to move between fields. Use of the "Enter" key will submit the form prematurely.
You should receive e-mail confirmation of this transaction within 5 days of submission. If you do not receive that confirmation, please request confirmation at orms@orms.org.

Your preliminary listing of sites will be retained by ORMS and will NOT be disclosed to the programs in order to keep your choices confidential.

This is not your list for matching purposes. You must also submit a Matching Form. The Matching Form is available on the "Forms" page and must be submitted by March 6, 2009 by all candidates.

The sites are listed below by state. Residencies listed with school acronyms in their titles are on-campus positions.

The first five program choices are included in your application fee. For each additional program (over five) listed on your application, there is a $10 per program surcharge. Also, for each program requested after submission of your application, regardless of the number of sites you originally listed on your application, there is a $10 per program surcharge.

Before submitting your application, please PROOF READ it carefully. This form is forwarded to program coordinators as it is received by ORMS, the exception being that we delete your OE Tracker number. We cannot make corrections to your application (i.e., add capital letters and necessary punctuation to your name where needed, arrange names in the requested order of Last name, First name Middle initial, fill in your optometry school or GPA, or make other necessary corrections). Also, we cannot process applications that are received without OE Tracker numbers.

Please use the "submit application" button only once. Multiple forms will be sent if you use the submit button more than once.
