| | This is the page for you to rank (#1 is most preferred) your
residency site selections. We have allowed up to ten choices for you.
The normal number of selections is 2-5 positions. If you choose only one
site, you are limiting your opportunities to match, but this is your ranking
list and any number up to 10 is permissible. This form is for your use
after making your final choice of desired residency sites.
- You must submit a Matching Form to ORMS in order to participate in the
match.
- Do not neglect the last question about the "Pool" List.
Should you not match at your selected programs, your name and demographic
data will be sent to programs with unfilled positions if you select "yes."
By responding positively, you would enhance your chances of being selected
for one of the unfilled positions
- Please do not submit ranking lists with no preferences as that type of
submission serves no purpose.
- Should you choose not to submit a ranking list, please notify ORMS at
orms@orms.org of your wish to withdraw
from the match. If you do not submit a Matching Form, ORMS cannot
match you to a site.
- Please wait until you have interviewed to submit your ranking list.
Duplicate matching forms create extra work, delay notification and may
create errors.

matched site.

Deadline for
Matching Form submission, close of business, 5PM CST, March 6, 2009.
Use the "Tab" key and "Shift-Tab"
key combination to move between fields. Use of the "Enter" key
will submit the form prematurely.
